What Do I Need to Apply?

You will need to provide the information listed below for all members of your applicant household: 
  • Email Address.  
  • Telephone.
  • Your mailing address including the correct zip code.
  • Estimated annual income for your combined household.
  • Social Security numbers or Alien Registration Numbers for all members of your household.
  • Dates of birth for all members of your household.
  • Race, Ethnicity, and Disability status.
