Reasonable Accommodation Forms and Policy

There are two main reasonable accommodation forms that the BHA has available. These are not required but can help with the processing of your request

• The “Request for Reasonable Accommodation” form allows you to state what accommodation you need and why you need it, and has a release for the BHA to speak with your medical professional or qualified service provider (for example a social worker or case worker) who has knowledge of your disability for verification of your needs.
• “Certification of Need”. This form is to be filled out by your medical professional or qualified service provider for verification of your needs.

There are differnt versions of these forms for applicants/Public Housing residents and Leased Housing participants.  

The Request for Reasonable Accommodation forms are also available below in larger font for clients with visual impairments. If a format other than the one provided below is required due to a disability, you may request that one be provided that meets your needs.
The Request for Reasonable Accommodation forms are also available in Spanish and Chinese below. Please note that these versions are for reference only. BHA will only accept completed and signed English versions.
