Reasonable Accommodation Forms and Policy

There are two main reasonable accommodation forms that the BHA has available. 

  • The “Request for Reasonable Accommodation” form allows you to state what accommodation you need and why you need it, and has a release for the BHA to speak with your medical professional or qualified service provider (for example a social worker or case worker) who has knowledge of your disability for verification of your needs.
  • “Certification of Need” (with verification source cover letter). This form is to be filled out by your medical professional or qualified service provider for verification of your needs.

There are several version of these forms. Find the correct reasonable accommodation form for your situation based on whether you are an applicant, Public Housing resident or Leased Housing participant.  

These forms are also available below in alternate format for clients with visual impairments. If a format other than the one provided below is required due to a disability, you may request that one be provided that meets your needs.

Note: The Request for Reasonable Accommodation form is also available in Spanish and Chinese below. Please note that these versions are of the applicant/resident forms and are for reference only and that the Boston Housing Authority will only accept completed and signed English versions.

Request for Reasonable Accommodation
Request in Alternative Format - for those with visual impairments
