General Description: Under the general supervision of the Program Director for Resident Services for the Elderly & Disabled Housing Program, provides training and support for Housing Managers, Office Support Staff and Custodians around client issues that are impacting their ability to live independently. Performs resident assessments and makes appropriate referrals to service providers; coordinates services to maximize service to our diverse resident populations. Have knowledge of local social services delivery system for elderly and disabled populations.
Duties & Responsibilities: - Collaborates with the property manager and on site staff to address the needs of tenants at risk of eviction due to lease violations resulting from mental illness and/or other behavioral problems and to assist tenants in maintaining tenancy where appropriate.
- Provides assessment, outreach, crisis intervention advocacy to tenants and linkages to necessary services and provide technical assistance to property managers.
- Provides linkage for our most frail, homebound elderly disabled residents.
- Attends task force meetings upon request and meets regularly with managers to discuss referral and other tenant initiatives.
- Assists tenant task forces and managers in bringing services (i.e. health, nutrition, transportation, etc) to the site by working with community-based and citywide agencies who provide these services.
- Assists task forces in organizing social, recreational and cultural activities for the residents.
- Attends task force meetings upon request and meets regularly with managers to discuss referral and other tenant initiatives.
- Under the supervision of Program Director for Resident Services establishes a Human services team consisting of local social service providers from appropriate service delivery systems, along with the property managers, who will meet on a regular basis to provide necessary assistance in accessing specific needs of elderly and disabled.
- Performs other duties as assigned.
Minimum Qualifications & Experience: Bachelor’s Degree or equivalent with at least 2 years experience working with elderly and disabled. Excellent communication skills required. Experience with mental health emphasis on substance abuse. Ability to work in a public housing settings nights and weekends if needed.
